I authorize Better Express LLC and its screening providers to contact current and previous employers, carriers, contractors, schools, and references to verify employment, safety performance history, accident history, and driver qualification information.
DOT drug and alcohol history release
I authorize previous DOT-regulated employers and their agents to release drug and alcohol testing history, refusal, SAP, return-to-duty, and required safety-sensitive employment information to Better Express LLC.
MVR and license verification
I authorize Better Express LLC and its screening providers to obtain motor vehicle records, CDLIS/license information, medical certification status where available through licensing records, and related licensing authority records.
PSP disclosure and authorization
I authorize Better Express LLC and its screening providers to access the FMCSA Pre-Employment Screening Program for pre-employment screening, including crash data from the previous five years and inspection history from the previous three years.
Federal FCRA Summary of Rights
I acknowledge that I have been provided, or given the opportunity to review, save, or print, the federal Summary of Rights under the Fair Credit Reporting Act.
FCRA disclosure
I understand Better Express LLC may obtain consumer reports or investigative consumer reports about me for employment, qualification, safety, compliance, retention, or legitimate business purposes where legally permitted.
Background report authorization
I authorize Better Express LLC and its screening providers to obtain employment-related consumer reports or investigative consumer reports where legally permitted, including additional reports during employment or contract review if allowed by law.
Clearinghouse limited query consent
I consent to limited FMCSA Clearinghouse queries by Better Express LLC and understand a full query requires separate specific electronic consent inside the Clearinghouse before detailed violation information is released.
Application certification
This certifies that this application was completed by me, and that all entries on it and information in it are true and complete to the best of my knowledge.
Electronic records and signature
I agree to complete this application electronically and understand my typed legal name is intended as my electronic signature.
Privacy acknowledgement
I understand this application will be stored in Better Express systems and used for recruiting, qualification review, compliance, and hiring follow-up.
